It is the responsibility of the parent/guardian and/or the financial guarantor to verify that their specific medical plan is in-network. If the insurance plan denies payment for "out-of-network" provider, the financial guarantor will be responsible for the full payment as allowed by your specific insurance plan. All known expenses, such as copays, coinsurance, or unmet deductibles must be paid in full before the appointment!
