How Do I Become A New Patient?

Please call   210-487-9668   during routine office hours to request a new patient intake packet. You will be sent a link from Intake Q where you can complete the forms securely online, upload the required documents (see below), and then submit them. Once this is completed, you will be contacted within 1-2 business days to schedule the 2 separate 1 hour intake appointments  (the  first one is with  parents/guardians only, and the  second one  also includes the child/teen, though for a very young child, or other instances these may be combined into a single appointment). At this time, all appointments are conducted via  Telehealth.


All payments are DUE IN FULL prior to each appointment! (Confirm that your specific medical insurance policy includes PMHP/Dr. Flynn in-network).

You will also be asked to attach some required documents directly into the intake packet...

--Photo ID (driver's license, etc) of  each parent(s)/guardian(s) who will "accompany" the child or teen during appointments.

--Photo of the FRONT and BACK of your child's primary (and secondary, if applicable)  insurance cards

--A copy of any  divorce/custody documents (if applicable)
